Admissions Overview

Application Procedures and Check List
St. Anthony Junior-Senior High School
Grades 7-12    SY 2011-2012 

1. Complete the application form- please type or print clearly.

2. Return completed application with $300 non-refundable fee(subject to change without prior notice) and a copy of student’s Birth Certificate to the Director of Admissions at the above address.

Please make check payable to St. Anthony High School, and indicate clearly “Application Fee for student’s name)”.

3. Have two (2) teachers complete the Teacher Reference Forms.  Please provide teachers with a stamped addressed envelope to be mailed directly to the Director of Admissions.

4. Have Principal or Counselor complete the recommendation form.  Please provide the Principal or Counselor with a stamp addressed envelope to be mailed directly to the Director of Admissions.

5. Complete the Consent for Release of Information.  Mail the completed form to the Director of Admissions.

6. Request your student’s present school to mail an official transcript of grades, a copy of a current report card, and current test scores to the Director of Admissions.

7. Applicants are required to take a placement test. Please call our Director of Admissions, at 244-4190 ext. 232 for dates and times.                                 

8. A personal interview with the prospective student is required of every applicant after the application has been submitted and the placement test has been taken..

9. Parents and students will be notified by mail of the school’s decision.

10. Students who have been accepted may be  required to have a physical exam before the start of the school year.  Immunizations must be current as required by the State of Hawaii Department of Health.

11. Financial Aid forms must be completed and returned by March 15, 2011.
